Institution Order Form
Records the institution legal entity, authorised signatory, selected plan, term, seats, fees, GST, billing/renewal method, included features, support level, applicable policy versions and signatures.
Provider Agreement
Records provider identity and verification, service category, platform fees or take rate, payout prerequisites, invoicing, content/data obligations, dispute handling, term and signatures. It does not make CatalystBox a party to the institution-provider service contract.
Funder Licence
Records the programme, licensed users, permitted reporting/data access, institution-identity permissions, fee, term, confidentiality and any impact-reporting conditions.
Data Processing Addendum
Records controller/processor roles, documented instructions, data subjects/categories, purpose, duration, confidentiality, security controls, subprocessors, incident assistance, rights requests, return/deletion and audit cooperation.
Diagnostic Data Addendum
Records the diagnostic source, explicit participating-user/institution permission, score fields, purpose, recipients, retention, withdrawal/disconnection method and security restrictions. Diagnostic data cannot be connected merely because an account exists.
